Support for family and friend carers at Dolphins
It would help us greatly to be aware of anyone providing regular care in the home. A carer is someone who provides help and support for a relative, friend or neighbour who cannot manage at home without your help.
You may be caring, giving practical, personal, emotional or financial support, for someone close to you who has a long term condition or is frail, memory loss or dementia, mental health, a child with special educational needs, or disability, or an autistic adult, or an adult with a learning disability.
What’s available for you?
Caring can often bring practical problems, stress and isolation. We feel that it is important that your GP practice knows if you are a Carer. By adding this information to your records it allows the practice to take into account, that you are a family or friend carer.
If you are a patient with us, register with us as a family and friend informal or unpaid carer.
If you tell us you are caring for someone, we will add this to your records.
We then will be able to contact you with any carer specific information that might be helpful in the future.
You can let us know when you next speaking with someone at the practice, by completing a self-registration form in the reception area room, or self-registration form on our website, link at the bottom of this page.

Carer Lead at Dolphins
We have a Carers Lead at Dolphins who can let you know about support for you and information and service that may support you in your caring role.
Carers Support Information
We have a board in our reception area with information for Carers, on our website and social media. Information relating to specific caring roles is available through the Carers Lead.
Carer Awareness at the Practice
Carer Awareness is included in staff inductions, staff training or information sessions once a year. The person you are speaking with may ask you if you are caring for someone, and can ask for the Carer’s Lead or other support to get in touch with you.
