Stay safe and healthy abroad
If you are travelling abroad you may need to be vaccinated. Before we make an appointment you need complete a travel questionnaire:
- you can download a form here or
- you can pick up a copy from reception or
- you can complete the online form below.
Please return printed forms to the practice in person or via email to [email protected] and one of our nurses will check what vaccines you require. You will be contacted by us to arrange an appointment. No appointments will be made without the travel questionnaire being completed.
It is important to make this initial appointment at least eight weeks before you travel so that the vaccines can be ordered. You will need a second appointment to receive the vaccinations. Your second appointment should be at least two weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription which cost more than the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
Travel vaccinations are not available on an NHS prescription.
