If you are assigned to us by the CCG please call/email the practice and we will send you the forms. Please see below the information you will need to bring to the surgery when you register.
Please complete the forms and bring them in with you on your first visit to the practice together with proof of identity and residence.
We ask that you provide us with your NHS Number. You may also sign the form on behalf of your immediate family members. Please do not delay registration until you need to see a doctor urgently.
As part of the registration process you will need to show us:
- Proof of Identity: Passport or EU ID card, Photographic UK driving licence (updated address), UK birth certificate
- Proof of address, (dated within the last 3 months): utility bill, rent book, bank statement, council tax bill
You will be asked to complete a Health Questionnaire. If you can please:
- Use our Blood Pressure Machine
- Use our Weight / Height machine to check your Body Mass Index
Patients will be allocated a doctor upon registration, if you have a preference please tell us and the request will be considered but may not always be appropriate.
