Repeat Prescriptions:

Any long-term or frequently required medication can be requested on a repeat prescription basis. Your doctor will set a review date to discuss your medication and the illness for which you take the medication.

In order to request a repeat prescription just hand in the tear off portion on the right of the prescription.

You can request a repeat prescription by:-

Placing it in the basket in Reception
Post it to the Surgery
Fax it to the surgery on 01444 451532
We cannot accept requests over the phone

Please allow two working days for the prescription to be ready.

If you wish to have the prescription posted back to you, please enclose a stamped self addressed envelope. Alternatively most of the local chemists will collect prescriptions from the practice. Please indicate on the request form which chemist you wish to collect the prescription. The review date will appear on your repeat prescription form.
