Registering as a patientRegistering With The Dolphins Practice

If you live within our practice area please ask reception for a registration pack, alternatively you may download our new patient registration pack here. This zipped file contains all the forms you need to register with us as a patient; fill the forms out and bring them in with you on your first visit to the practice together with proof of identity and residence, as stated below:

We ask that you provide your NHS Number to help with registration although we can accept patients without it. You may also sign the form on behalf of your immediate family members. Please do not delay registration until you need to see a doctor urgently.

- FAST Score Questionnaire
- Consent to leave Telephone Messages

As part of the registration process you will need to show us:

  • Proof of Identity: Passport or EU ID card, Photographic UK driving licence, UK birth certificate
  • Proof of address (dated within the last 3 months): utility bill, rent book, bank statement, council tax bill

You will be asked to complete a Health Questionnaire. If you can please:

  • Use our Blood Pressure Machine
  • Use our Weight / Height machine to check your Body Mass Index

We would like you to book a New Patient Health Check, if you are aged 45 plus.

Please book an appointment as soon as possible after registration. Your appointment will be with our Health Care Assistant who has been specially trained to give Healthy Lifestyle and Disease Prevention advice and who can support you in accessing specialist services.

You will be registered with the practice and you may book an appointment with any doctor. However, for continuity of care, you will be appointed a ‘usual Doctor’. Please tell us if you have a preference. Requests will always be considered but may not always be appropriate.

Change of Name and/or Address

Please advise us of any change of name, address or telephone number at your earliest convenience to enable us to keep our records up to date and to contact you should the need arise. If you move out of the practice area, we may ask you to change to another practice nearer your new address. You can update your personal details by downloading our change of address form.

Please remember - we are unable to contact you if we do not have your up to date information.

Practice Boundary - Proposed Changes

The proposed changes to Practice Boundaries and the ability to register with a GP of choice, as announced in the press on 17th September 2009, have not been formally adopted and therefore the Practice Boundary remains the same. The proposed changes are suggested to be implemented within 12 months but have not been formalised. We will amend this page as appropriate.

Each NHS Number is a unique 10-digit number which is printed on your NHS medical card or will from now on be written on any communication you have had from the NHS.
